How Does Person Centered Planning Work?
Our trained and experienced planning facilitators assist people with developmental disabilities to create a plan in collaboration with their support network. This plan becomes the framework of their supports. A support plan helps people explore the things that are most important to them in their life today. The facilitator will guide the process and assist in the creation of meaningful and concrete goals which help to guide the person and their support network.
The development of a plan helps people to see a more positive future. The planning process helps people to develop goals and a plan on how they can be achieved. Person Centred Planning can also be extremely helpful to support professionals, families and other members of the support network to clearly understand the wants and needs of a person with a disability.

Community Living London planning facilitators are experienced in using a variety of tools to assist people with disabilities and their families to create a plan. At the end of the process, you will have a plan which will provide clarity, establish concrete goals and see a more positive, possible future.

Planning services can be accessed through Developmental Services Ontario.
